New Patient Forms

new patients

If you are seeing Dr. Bhela for the first time, you might be wondering what to expect at first visit

  • Please bring your photo ID, insurance card(s), and a list of current medications and dosages.
  • If you have had an x-ray, MRI, or CT Scan for your condition, please bring these pictures (not just the report). If x-ray picture was not taken properly, we may need to take x-rays here in our office to provide the best picture to accurately diagnose your condition
  • Upon arrival, you will be given a set of forms to fill out if you have not done so already (Why not go ahead and do it now?) Just click on New Patient form.

  • Once your paperwork is filed in, a staff member will take you to an exam room. She’ll have you settle in, remove your socks and shoes and ask a few questions
  • Sit back and relax in your exam chair. Dr. Bhela will be with you in a few minutes.
